Senior Blue HMO

With our Medicare Advantage Health Maintenance Organization (HMO) plans, you can choose from two $0 monthly premium plans or low monthly premium plans, as well as plans with or without prescription drug coverage and access to care out-of-network. Every HMO plan offers covered preventive services, worldwide emergency care and nationwide urgent care coverage, no deductible and no referral.

Forever Blue Medicare PPO

The Forever Blue Medicare PPO offers a low monthly premium, worldwide emergency care and nationwide urgent care coverage, no deductible, no referrals, prescription drug coverage, and the freedom to see any doctor who accepts Medicare.

Bluesaver Medical Savings Account (MSA)

Our BlueSaver MSA is a high deductible Medicare Advantage Plan that offers a $0 monthly premium, nationwide urgent care coverage, the freedom to see any doctor who accepts Medicare, and gives you more control of your health care costs with the ability to save unused health care dollars for future medical expenses.

BlueCross BlueShield Rx PDP is a stand alone Medicare Part D prescription drug plan.  It offers members a cost savings with preferred pharmacies, convenient mail-order service, and a nationwide network of pharmacies.

Optional Supplemental Dental Benefit

The optional supplemental dental benefit can be added to your Senior Blue HMO or Forever Blue Medicare PPO plan for $11 per month.  This optional benefit is not covered by Original Medicare or included in the benefit package of your plan.

You must live in one of the following Western New York counties to be eligible for enrollment in one of our Medicare Advantage plans:

  • Alleghany
  • Genesee
  • Cattaraugus
  • Niagara
  • Chautauqua
  • Orleans
  • Erie
  • Wyoming

You may enroll in a Medicare Advantage plan only during specific times of the year:

  • When you first become eligible for Medicare, the seven month period  beginning three months before you turn 65,
    the month you turn 65 and three months after you turn 65
  • If you get Medicare due to a disability
  • During the Annual Enrollment Period from October 15, 2014 and December 8, 2014

In most cases, you must stay enrolled for the calendar year that starts the date your coverage begins.

In certain situations, however, you may be able to join, drop or switch a Medicare Advantage Plan, excluding BlueSaver MSA, at other times. 
Some of these situations include the following:

  • During the Medicare Advantage Disenrollment Period, between January 1 and February 14, you may drop your Medicare Advantage Plan and return to Original Medicare (does NOT apply to the BlueSaver MSA plan)
  • If you move out of the plan’s service area
  • If you have both Medicare and Medicaid
  • If you qualify for extra help to pay for your prescription drugs
  • If you live in an institution

Please note that a plan may not be available to beneficiaries the following contract year because, by law, plan sponsors can choose to not renew their contract with CMS, reduce their service area or CMS may refuse to renew the contract, resulting in a termination or non-renewal of your plan.
